1) if the clutch bearing is not pressed on to the shaft (item #16 in the attachment pictures) fully, it will not have the correct gap to the diaphragm and the adjustment bolt may not be able to compensate properly.
2) if you are not using a genuine Honda part, the aftermarket part being used may be wider/deeper which causes it to be too close to the diaphragm. Again, the adjustment bolt may not be able to compensate properly.
3) is the shaft which the throwout bearing is pressed a genuine Honda part? If not, it may be too long.
4) is the diaphragm plate a genuine Honda part? If not, the height may be greater than the stock part.
And if you ask "what is the correct gap?", my answer would be a gap which is enough so the throwout bearing is not touching the diaphragm plate when the clutch pedal is out. 1 to 2mm is all it takes.
And this is the gap between the bearing and the plate. Enough gap so it is not in constant contact.
